Team Registration Forms*

 

Please click the buttons below to access, print, and complete all necessary forms for your child's registration.  Separate forms must be completed for each Team Member regardless of family affiliation and must be on file 15 days prior to the first scheduled team practice.  Failure to complete and return forms in a timely manner will result in your child(ren) being unable to participate in activity during practice.

IESA Physical Card must be completed for Competition Team Members

 

*Additional forms must be completed by Competition Team Members which are available in hard copy only.  Birth Certificates must be submitted for all participants on the Competition Team and the Pearls (PreK - K) Team.
